Founder, Medical Director
J Carvel Jackson, DO
Dr. Jackson received his osteopathic medical degree from College of Osteopathic Medicine of the Pacific in 1988. He completed residency training in Physical Medicine and Rehabilitation at Stanford University in 1992. He is board certified and a fellow of the American Board of Physical Medicine and Rehabilitation and the American Osteopathic Board of Rehabilitation Medicine. In addition to this, Dr. Jackson he also received certification as a palliative medicine expert from Harvard University. He is the founder of Addiction Navigators and has been the Medical Director for prestigious outpatient clinics and hospitals in valley. He has served as the Medical Director of John J Rhodes Rehabilitation Institute, HealthSouth East Valley Rehabilitation Hospital and Honor Health Rehabilitation Hospital. The physicians of Arizona have recognized his skills by voting him a “Top Doc In Phoenix” multiple times. He founded PalliCare Health to bring specialized palliative care and chronic pain management to Arizona patients in an outpatient setting.
Dr. Jackson is dedicated to assisting those struggling with substance use disorders (alcohol, prescription and street drugs), chronic pain and disability.

Brooke Jackson
Brooke Jackson is a medical device professional with over 15 years in device and dental sales, and a business owner with an entrepreneurial track record built on that same experience. She holds a degree from Utah State University, where she competed as a collegiate athlete. As co-founder of PalliCare Health, Brooke leads business development and operations for the practice, drawing on a career spent building referral networks and strategic partnerships across healthcare organizations.
For Brooke, this work is personal. She is proud to be on the same team as her father, united by one mission: helping people find relief from chronic pain and disability, and a better quality of life.
